Visa Forms

A complete listing of the required forms is available at http://www.ttsusvisas.lk.  All applicants must bring a completed Form DS-156 to their interview.  Many male applicants require the Form DS-157 and other applicants may require the Form DS-158. 

• Electronic Visa Application Form--must be filled out online at http://evisaforms.state.gov.  Required for all applicants, printed on white photocopy quality paper

• Supplemental Non-Immigrant Visa Application DS-157 (required for male applicants between the ages of 18 and 45 years old)

• Contact Information and Work History for Non-Immigrant Visa Applicants DS-158 (required for certain job-related applicants)

Note: If you cannot open the DS-157 and DS-158 simply by clicking on them, we suggest you right-click and select "SAVE."  Save the file on your computer and then open the file directly from there.  We recommend using Adobe Acrobat versions 5.0 and newer to view these documents.  Download the Latest Version of Adobe Acrobat Reader.  Visually disabled can convert PDF-documents with Adobe Simple Form.
